Please feel free to fill out the New Patient intake Form for your first visit. The stress assessment form and health appraisal-brief form are also helpful to bring completed to the first visit. If you are bringing your child in for the initial examination please complete the health appraisal-kids form!

Dr. Johnson is pleased to offer flexible hours. Normal hours are as follows:
- M/W/Th — 8-6
- Tuesday — 8-12
- Friday — 8-1
- Saturday by appointment only!
If you cannot make an appointment during these hours please call the office and another time will be arranged. Your health is our priority!
